Bone grafting for dental implants NYC

When placing dental implants, it is critical that the jaw bone is sufficiently strong enough to hold the metallic implant.  Prior to placing any dental implants, it is important for a thorough evaluation of the bone density, or strength.  This is done through clinical examination as well as x-rays.  When it is determined that the bone is not sufficiently strong enough to hold a dental implant, a bone graft is often indicated.

 

Previously, before the use of bone grafts, some patients were not candidates for dental implants.  If we diagnosed them with bone loss, such that the bone was not sufficiently strong enough to hold the implant, we did not place the implants, and alternatively, we would use a fixed bridge or a denture.

What is a bone graft for a dental implant?

It is not uncommon for patients to need a bone graft before implanting a dental implant.  The loss of teeth that requires a dental implant goes hand-in-hand with the reasons why one would lose bone density, or have diseased bone in the jaw.  Bone grafting is a common procedure that is done with dental implants for patients that do not have sufficient amount of healthy bone to support the dental implants.  A bone graft is used to make the jawbone stronger in order to be capable supporting the dental implant.

 

A bone graft is minor surgical procedure that is performed in the dental office.  When we determine that a patient needs a bone graft, we are able to restore the strength of the jaw bone by implanting bone tissue in order to re-creating the jawbone that is strong enough to be supportive for the implant.

 

A small incision is placed in the gums in order to access the bone where the bone graft is necessary, and where the implant will be placed in the future.  The proper amount of bone material is placed in order to build up the jawbone to strengthen it sufficiently.  A bone graft may be made out of patient’s own bone or artificial bone.

 

We often use artificial bone called an allograft.  While a patient’s own bone may have slightly greater chance incorporating, an allograft also incorporates excellently. By using an allograft, we do not need to make a separate incision, and have a separate surgical site that needs to heal in order to harvest the bone.  This reduces the number of places that need to heal and the time it takes to heal.  It also reduces the amount of pain possibly experienced after this minor surgery.  After we place the bone graft, the patient’s body will create its own new bone that will incorporate inside of, and around the graft in order to make it strong.

When is a bone graft necessary for dental implants?

Any patient that has lost teeth is a possible candidate for dental implants. Of these patients, when we determine that the bone is not strong enough to support the dental implant, then these patients are possible candidates for a bone graft procedure prior to a dental implant.

 

When chewing, the strong muscles of the jaw exert forces on the teeth. For this reason, it is imperative that the bone be solid enough to support the implant such that the implant does not move or fail in the future. We evaluate all patient’s prior to getting dental implants to make sure that the bone is sufficiently strong enough to support the implants. This is done through clinical examination and x-rays. We will be able to tell you at your initial consultation if you need a bone graft or not, prior to a dental implant procedure.

Who would need a bone graft for dental implants?

Prior to a dental implant procedure, we will evaluate every patient to determine the need a bone graft or not.  This is done through clinical examination and x-rays.  For some patients, we may determine that they a need a bone graft at the same time as their tooth extraction.  We do this in order to plan for the future where a dental implant may be necessary to replace the missing tooth.

 

For other patients, we may determine that you need a bone graft at the time of your dental implants.  We evaluate the bone density as well as the height, width and depth of the jaw bone.  For some patients, we may determine that the jaw bone has sustained significant damage such that the implant will not be fully supported.  If we feel that the jawbone will not provide sufficient foundation for your implant, we will recommend a bone graft at that time.

What causes bone loss?

Bone loss can occur due to many things:

 

 

While bone loss may occur as a result of disease, it can also result from the lack of forces on the bone or abnormal forces on the bone.  Bone remodels itself all the time.  For example, when one wears braces or Invisalign, we are able to move the teeth into better position by having the bone remodel while the teeth are moved using pushing and pulling forces.  After a tooth extraction the forces on the bone are reduced which can cause the bone to resorb or become weaker.  While this does not happen overnight, it can occur relatively quickly after a tooth extraction.

 

Periodontal disease can also cause bone loss.  This process occurs due to the build-up of bacteria that can erode the bone.  This occurs mostly in patients that do not have their teeth cleaned regularly.  Fortunately, sometimes a deep cleaning can reverse the process of this bone loss.  However, when the bone has lost some of its strength, it may not be able to support a dental implant.  For these patients, we will perform a bone grafting procedure prior to dental implants.

After tooth extraction, forces that are normally exerted on the bone in that area are reduced.  This results in weakening and loss of bone due to these reduced forces.  Studies have shown a significant loss of bone at the tooth extraction site as early as three years after the tooth extraction.

Injury to the jawbone may also cause loss of the jaw bone such that it is not sufficiently strong enough to support a dental implant.

How are bone grafts done?

The first step in getting a bone graft for dental implants is to have a consultation with our dental implant specialist. We will do a clinical examination and x-rays to determine if the bone is sufficiently strong enough to support an implant or not. If it is not strong enough, a bone graft will be recommended. Additionally, any underlying factors such as periodontal disease must be addressed prior to performing a bone graft for dental implants.

 

If we determine that you are a candidate for a bone graft for a dental implant, will schedule an in office minor surgical procedure in order to place the graft. Local anesthesia is utilized. This minor surgery starts with an incision in the gum in order to access the bone.

 

A small amount of the jawbone will be filed in order to remove any diseased bone and create a surface for the new bone graft to adhere to. The proper amount of bone graft will be inserted into the area in order to provide a sufficient size and strong surface for the implant to anchor to. Once the bone graft is in place, sutures are utilized to hold the graft in place and allow it to heal, and to adhered to the natural bone, as well as for the body to replace and incorporate the graft with new bone.

 

At Sachar Dental NYC, in addition to placing the bone graft, we also place a membrane over the bone graft. This greatly increases the success rate and incorporation of the bone graft into the jaw bone. In our office, we include the membrane in the cost of the bone graft and do not charge any additional fees for it.

 

After the bone graft is inserted, it takes approximately one month for the graft to become sufficiently strong enough to support a dental implant. We have our patients return to our office for an additional evaluation to make sure that the bone is strong enough which is done through clinical examination and x-rays.

What can I expect after a bone graft procedure?

After your bone graft procedure has been completed, you may experience minor discomfort, swelling and bruising in the area. For most patients, this procedure is relatively pain-free and perceived as more minor than it sounds. You will be instructed not to eat anything until the anesthesia wears off. We may prescribe antibiotics, anti-inflammatories and/or pain medications. At the time your visit, you will be given complete instructions relevant to your case individually.

Most PPO dental insurance plans cover the costs of bone graft for dental implants. The bone graft is usually covered at 80%. You will only be responsible for your percentage of the costs at the time of your visit. We will then submit an insurance claim to your insurance company, and collect payment from them directly.

Yes, you may be able to use your medical savings accounts or flexible spending dollars to cover charges related to your treatment. At the time of your service, we will provide you with all the necessary documentation and receipts.
