Causes of Tooth Pain

Cavities occur when a part of the tooth is rotted or destroyed.  When food particles remain on a tooth for an extended period of time, bacteria build up.  Bacteria release an acid that destroyed the tooth enamel and dentin.  When this process is deep enough, it can cause pain in the tooth and cavities can get very large.

Oftentimes severe pain in a tooth is caused by nerve inflammation.   Inflammation can be reversible or irreversible.  He can be caused by any kind of irritation to the tooth.  This includes tooth decay, trauma, fractures, and broken fillings to name a few.  In early stages nerve inflammation is reversible often.  In later stages, it can become irreversible and required a need for root canal or extraction.

Tooth trauma can result in broken teeth, hairline tooth fractures or full-thickness fractures of the tooth and fractures of fillings.  Trauma can occur from an acute episode, or from chronic grinding or clenching of teeth.

It is possible to get infections of both the teeth and gums. A gum infection occurs when bacteria builds up around the gums and spreads when untreated. A tooth infection is usually a result of nerve death within the tooth.

When wisdom teeth try come in and there is not enough space for them in the mouth to break through the gum, they can become very painful.

Gum disease occurs when bacteria builds up around the gums due to plaque accumulation from improper brushing. This causes the gums to become puffy and swollen and bleed easily.  Pain or discomfort may be associated with this.

Diagnosis of Tooth Pain

At Sachar Dental NYC, we always will listen to our patients account of their medical and dental history leading up to their recent painful episode. We then perform a thorough clinical examination in addition to evaluating any necessary x-ray imaging.

We are experts in complex diagnoses. Oftentimes pain in one tooth may be caused by referred pain from another adjacent tooth or other area of the mouth. Unfortunately, we have seen many patients undergo procedures for tooth pain that addresses the wrong tooth, and they still have pain after initial treatment. There are specific tests that we do to determine the actual cause of pain prior to any invasive procedures. Our philosophy is to always do the least invasive procedures and utilize the least toxic materials to relieve your tooth pain.

Additionally, we think in "wholistic" terms to account for the reason why you’re experiencing tooth pain and not just a single tooth. Pain may be coming from the misalignment of your bite or other generalized structural deficits that we will fix to ensure your tooth pain does not return.

Treatment of Tooth Pain

Once we have determined the cause of the pain, we formulate an appropriate treatment plan.  These plans always include the least invasive options that will relieve pain and prevent it from coming back. For each different cause of tooth pain, we have different procedures that we may use. For example:

For cavities we clean out the decayed part or the tooth and replace it with fillings. We use mercury-free and BP- free materials only.

For reversible nerve inflammation treatment includes addressing the trauma or pressures that cause it. For irreversible nerve inflammation we clean out the decayed part of the tooth and perform a root canal. A root canal is removal of the nerve and the infection and replace them deep within the tooth with filling material.

Broken teeth are repaired depending on the severity of the broken tooth. This ranges from repairing small breaks with fillings to repairing large breaks with crowns. Rarely, if the tooth is broken so severely that it cannot be saved, we perform a tooth extraction and preserve the underlying bone such that later we can perform a tooth implant if desired.

Tooth infections are treated with antibiotics and sometimes physical drainage.

Gum disease is treated by removing all the plaque and tartar and flushing with high pressure water. When needed we perform local antibiotic injections into the gums.
